Healthy Choices, Inc is a non-profit, 501 c-3 organization incorporated in the state of Oklahoma and is doing business as the Adventist® CHIP® Association. It is our intention to carefully protect the good name of each of the entities involved or implied in our name, therefore we have adopted the following disclaimers and restrictions:

Adventist® Name: The Adventist® CHIP® Association is an independent ministry that supports the mission of the Seventh-day Adventist® Church; however, it is not part of, affiliated with, or endorsed by the General Conference of Seventh-day Adventists® headquartered in Silver Spring, Maryland or any of its affiliates known as the Seventh-day Adventist® Church. Thus, any content or opinions expressed, implied or included in or with the services offered by the Adventist® CHIP® Association are solely those of the Adventist® CHIP® Association and not those of the General Conference or the Seventh-day Adventist® Church.

CHIP® Name: The name CHIP® is a registered service mark name owned by Dr. Hans Diehl, the owner of Lifestyle Medicine Institute (LMI) in Loma Linda, California. Dr. Diehl and LMI are under no circumstances liable for the content, conduct, or outcome of programs conducted by Adventist® church leadership in their respective program sites. Adventist® CHIP® leadership May not promote or sell concepts, services or merchandise that are not part of the Video-CHIP program except for CHIP®-compatible books, health foods, and lifestyle centers owned and operated by Seventh-day Adventist® entities. Active CHIP® leaders and staff will also honor the CHIP® name by practicing the CHIP® lifestyle principles. The CHIP® logo and knife and fork logo may be used only in conjunction with the full Video-CHIP® series. Since all CHIP® materials, including videos and pertinent materials are copyrighted, they cannot be duplicated without the express written consent of the International Nutrition Research Foundation, the not-for profit arm of Lifestyle medicine Institute.

Association Name: The Association is doing business under the non-profit entity, Healthy Choices, Inc, a 501 c-3 organization incorporated in the state of Oklahoma. It exists to (1) promote health education programs that encourage healthful lifestyle choices that are in harmony with the Seventh-day Adventist® Church, (2) provide leadership materials, training, and on-going support to Seventh-day Adventist® churches that are conducting or wish to conduct lifestyle health education programs for their members and surrounding communities, and to (3) achieve its objectives without discrimination on the basis of race, color, religion, sex, or nationality or ethnic origin.
